School Nurse

For the health and safety of all students, please keep your child home if they have:

• Fever: A temperature of 100.0°F or higher. Students must be fever-free for 24 hours without fever-reducing medication such as Ibuprofen (Motrin, Advil) or Acetaminophen (Tylenol).
   
• Vomiting or Diarrhea: Within the past 24 hours.
   
• Unexplained Rash or Draining Sores: A physician’s note is required upon return to school.
   
• Contagious Illness on Antibiotics: Students must be on antibiotics for at least 24 hours before returning for illnesses such as Strep Throat, Impetigo, or Conjunctivitis (Pink Eye). A physician’s note is required.
   
• Severe Cold Symptoms: Active coughing or secretions that cannot be controlled.
   

If a child becomes ill during the school day, families will be contacted and the child must be picked up. Please ensure that all Emergency Contact Information is kept current.

All students must provide:

• A Universal Child Health Record (Physical Form)
   
• An up-to-date immunization record

If your child requires medication during school hours (prescription or over-the-counter):

• A Request for Administration of Medication Form must be completed by a physician. This includes diagnosis, medication name, dosage, timing, and duration of treatment. Parents must also sign the form.
   
• Prescription medication must be in the original pharmacy-labeled bottle.
   
• Over-the-counter medication must be in the original packaging.
   
• Medications must be delivered to the nurse’s office by a parent/guardian or responsible adult. Students may not transport medication to or from school.

If your child has allergies that may require emergency medication at school:

• Please have your child’s physician complete the Administration of Epinephrine/Benadryl Form.
   
• Return the completed form along with the prescribed medication to the school nurse.
   
• Families are also asked to complete the Allergy Reaction Interview Form to provide additional details about your child’s allergic reactions.

Families of students with asthma should provide a completed Asthma Treatment Plan.

• It fulfills NJ Law N.J.S.A. 18A:40-12.8 (Physician’s Orders) and the District’s Request for Self-Administration of Medication.
